Courteous Canine, Inc - Sign Up Sheet

To sign up for a group class:

We must have on record proof of your dog's vaccinations, including Bordatella.  Please bring a copy of your dog's vaccinations including Bordatella or arrange to get it from your vet.

Call or e-mail Courteous Canine for the start day and time for the group class you have selected. All classes are $139.00 for eight one-hour (weekly) sessions. If you decide not to attend the class you have paid for and give us 72 hours' notice, we will refund your payment. (Bounced checks will incur a $25 charge.)

Group classes fill quickly and are limited to 8-10 dogs per instructor. Classes fill on a first-come basis, so sign up online or mail in your form and payment today. If you mail in your sign up, we will call you to confirm your spot in the class. If the class is filled, you may apply your payment to another class or we will refund it to you.

Please bring to the first class:

1. Your dog
2. Proof of current vaccinations including Bordatella
3. Lots of yummy treats your dog loves. These treats should be something special that will be given to your dog only during training sessions. Cut-up hot dogs, chicken, steak, cheese, or soft chewy dog treats are all good.
4. A buckle collar (no choke or prong collars), gentle leader or harness and a 4' or 6' lead. Please, no flexi or chain leashes.

Directions:

Courteous Canine, Inc. is located in Lutz, off of Country Line Road. From the South, take I-275 north and exit at Country Line Road. Take a left at the end of the ramp onto the 56. Turn Left (South) onto Cypress Creek Road, then right (West) onto Coutry Line Road, then go left (South) on Amanda Park Drive to Melissa Country Way.

From the North, from County Line Road, go south on Amanda Park Drive to Melissa Country Way.
